Good question. Traditionally, I think it was an insult aimed at males who didn't conform to 'masculine' stereotypes. I suppose it's when a male shows more gentle, submissive and more stereotypically feminine traits that don't fit the alpha male mould.
I don't think a person who identifies as a sissy has to be a crossdresser but, I suppose, many (like myself) are drawn to it. I certainly don't need to be in female clothes to be submissive or serve a Dom(me) - it just feels right.
At the end of the day, they're only labels.

Well I would say I’m a little sissy as I like domains and domaint girls as I am quite shy and have been classed as a feminine bloke. But the other side of it just doesn’t appeal to me but that’s my opinion and I have no problem with it. It’s just not for me
